What happens if I decide to stop testosterone replacement therapy?

Stopping TRT causes circulating testosterone to decline to pre-treatment levels or temporarily lower, often accompanied by a transient return of fatigue and low mood while natural production restarts. Many of the issues that brought patients to seek replacement will return.

When exogenous testosterone is discontinued, the body must restart its own hypothalamic-pituitary-gonadal (HPG) axis to resume natural testosterone and sperm production.

During the washout period, circulating testosterone levels will drop significantly before endogenous production recovers. Men commonly experience a temporary recurrence of fatigue, low libido, mood fluctuations, and reduced stamina.

The timeline and degree of natural recovery depend on factors including treatment duration, dosage, age, testicular reserve, and whether the underlying cause of hypogonadism was primary or secondary.

Risks and limitations

  • If you stop therapy, natural testosterone levels generally return only to your baseline pre-treatment level, meaning original deficiency symptoms may persist long-term unless underlying causes were resolved.
  • Discontinuation should always be managed under physician supervision to safely monitor the transition.
