This notice describes the mensmedicalinstitute.com website only. It does not describe how the practice handles medical records or information you give the office in person or by telephone.
What the website asks you for
When online requests are available, the Request a Call form asks for your first name, last name, email address and telephone number, and requires you to confirm that you agree to be contacted. Those four details and that confirmation are the only information the form accepts.
The form does not ask for, and should not be used to send, symptoms, diagnoses, medications, laboratory results, insurance details, card numbers or any other medical or financial information.
Reading the rest of the website does not ask you to type in any information. As with any website, the hosting and security providers that deliver these pages may process technical details of your visit, such as your network address and browser. What those providers keep, and for how long, has not yet been confirmed by the practice and is being verified.
What happens to a Request a Call submission
The Request a Call form appears only when a secure delivery connection to the practice's lead system is configured. When it is not available, the website asks you to telephone the office instead of collecting your details.
When the form is available, the contact information you submit is sent to the practice's secure lead system and is used to respond to your request. The website's own application code does not save those details in its database and does not deliberately write them to its logs. What the underlying hosting and security providers may record about a request has not been independently confirmed by the practice and is being verified.
Cookies and measurement
No third-party analytics or advertising service is configured on the public pages of this website at this time, and the application code does not set advertising cookies.
The staff area of the website uses sign-in credentials and session storage in your browser so staff members can stay signed in. That applies only to authenticated staff, not to visitors.
