Who is a candidate for medical weight management with GLP-1 medications?

Candidates generally include adults with a BMI of 30 or greater, or a BMI of 27 or greater with at least one weight-related condition such as hypertension, dyslipidemia, or prediabetes.

FDA criteria and clinical guidelines establish that GLP-1 receptor agonists are indicated as adjuncts to a reduced-calorie diet and increased physical activity for chronic weight management in qualifying individuals.

Candidates typically present with a body mass index (BMI) of 30 kg/m² or higher (obesity), or a BMI of 27 kg/m² or higher (overweight) accompanied by weight-related comorbidities such as elevated blood pressure, insulin resistance, or high cholesterol.

A physician evaluation is essential to confirm candidacy, evaluate metabolic parameters, and screen for contraindications such as a personal or family history of medullary thyroid carcinoma or multiple endocrine neoplasia syndrome type 2 (MEN 2).

Risks and limitations

  • GLP-1 medications are contraindicated in individuals with a history of medullary thyroid cancer, MEN 2, or prior severe pancreatitis.
  • These medications are intended to complement, not replace, structured nutrition and strength training.
